the role for you
An exciting opportunity is available within Adult Care, Housing and Public Health, as an Assessment and Review Coordinator working in our First Contact Team.
The post is based within the 'Front Door' of Adult Social Care where the focus is to ensure that adults of Rotherham and their carers are enabled to prioritise their well-being, maximise assets, promote independence and ensure vulnerable adults are safeguarded appropriately when needed.
As an Assessment and Review Coordinator in the First Contact Team you will be responsible for screening referrals and enquiries over the telephone and providing a response to our customers who may attend Riverside House.
You will support customers to maximise their independence and personal outcomes through the provision of comprehensive advice, information, practical support, and signposting.
Focusing on strengths, prevention, early intervention, and the promotion of wellbeing throughout your practice. You will ensure that customers receive the right support, advice, and information at the time they need it.
We offer a genuine culture of commitment to empower our customers and support them to live as full and independent life as possible within the community.
You may be completing referrals to access short term support or assistive technology to help people live independently in their own homes.
You will work as part of a wider team and work closely with Wellbeing Advisors, Social Workers, Occupational Therapy colleagues and Team Managers.
