We are excited to announce a new role based in the London Borough of Merton with a focus on supporting young people to transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Services.

The post holder will also support families and work alongside teams and liaise with wider organisations ensuring wider system collaboration to wrap around support for our young people.


These new innovative roles have been developed to ensure our young people moving from CAMHS services into Adult services are sufficiently prepared and supported to navigate adult provision and networks, whilst maintaining their progress and promoting independence.


The aim of the CAMHS Transition posts is to support the successful transition of young people aged 17-25 into appropriate mental health services and to enhance the social and mental health outcomes of young people experiencing difficulties as they grow into adulthood.
