Discharge Coordinator

We are looking for a Band 4 Discharge Coordinator to join our team on the Surgical Emergency Unit at the John Radcliffe Hospital.
As well as meeting with patients and their families, you will liaise with internal and external stakeholders and will be involved in finding solutions to complex discharge issues.
- Act as a central point for all enquiries relating to patient discharge planning within the SEU Wards
- To work closely with the Surgical Navigator and SEU Bleep holder in the management of the discharge process.
- To work as a member of the multidisciplinary team to support the discharge process.
- Assist in the process of assessing and making appropriate and timely discharge plans for patients on the ward.
- Communicate effectively information to Patient/relatives/carers and all members of the multidisciplinary team.
- Encourage the patient's family and friends to be actively involved in the discharge process.
- To record patient discharge information within the nursing discharge assessment form
- To input patient discharge plans on EPR/ case notes
The Trust comprises four hospitals - the John Radcliffe Hospital, Churchill Hospital and Nuffield Orthopaedic Centre in Headington and the Horton General Hospital in Banbury.

We are looking for pro-active, highly motivated individuals to work autonomously or as part of a team. The applicant will be expected to be enthusiastic, organised with excellent communication and IT skills.They should be able to use their own judgement to resolve complex issues.
They should be able to work well under pressure and be able to prioritize their own workload under constantly changing demands.
