Job Overview

We are seeking a highly skilled and experienced healthcare professional to lead our Discharge Service, working across all Acute sites and Short Stay Hub Beds as part of the system integrated Transfer of Care Hub.

Main Responsibilities
  • Lead the Discharge Liaison Nurse Team and Liaison Hub Team, providing expert assistance to clinical specialties to support effective discharge planning.
  • Develop and implement complex discharge planning strategies, working with internal and external partners to achieve safe and timely discharge.
  • Collaborate with multidisciplinary teams to provide expert liaison with external community service providers, the Local Authority, Community Trust, and Voluntary sector.
  • Enhance skills and knowledge in complex discharge management and coordination, and develop negotiation skills to achieve optimal outcomes.
  • Contribute to the continued development of the Discharge model from the acute hospital, participating in service development work.
