Job Summary

We are seeking a skilled professional to join our team as a Discharge Coordinator. The successful candidate will play a vital role in supporting the discharge process and ensuring a smooth transition for patients from acute beds to Hub beds.

Key Responsibilities
  • Work collaboratively with the multidisciplinary team to identify patients who require Hub bed placement and facilitate the discharge process.
  • Assess patients' needs and develop discharge plans that ensure continuity of care and maximize independence.
  • Communicate effectively with patients, relatives, carers, and healthcare professionals to ensure seamless transitions.
  • Liaise with other providers to ensure proactive management of discharge plans and ongoing support.
  • Record and input patient data and information onto multiple IT systems.
  • Work under the direction of Senior Nurse Teams to prioritize discharges on a daily basis.
  • Develop and implement programmes of therapeutic activity for patients, in collaboration with OT and Physiotherapist.
Requirements
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Excellent organizational and time management skills.
  • Knowledge of healthcare systems and processes.
  • Ability to work collaboratively as part of a multidisciplinary team.
