Job Summary

We are seeking a highly skilled and experienced nurse to join our team as a Band 7 Community Hub Matron. This is an exciting opportunity to work in a dynamic and fast-paced environment, leading the discharge planning process and ensuring seamless transitions for patients.

Key Responsibilities
  • Lead the discharge planning process, working closely with multidisciplinary teams to ensure timely and effective discharge of patients.
  • Develop and implement discharge plans, taking into account individual patient needs and preferences.
  • Collaborate with community services, social services, and other stakeholders to ensure a smooth transition of care.
  • Provide education and support to ward staff on discharge planning and related topics.
  • Contribute to the development of policies and procedures related to discharge planning.
  • Participate in quality improvement initiatives to enhance the discharge planning process.
Requirements
  • Registered Nurse with specialist training or experience in discharge management.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and analytical skills.
  • Computer literate with experience of using electronic patient records systems.

Working Arrangements

This is a full-time position, working 37.5 hours per week. The successful candidate will be required to work a rota, including evenings and weekends.
