Social Worker

We are seeking a skilled Qualified Social Worker to join our Hospital Discharge Team within the Social Care industry in Oxfordshire County Council.
Key Responsibilities:
- Progress cases of service users from Adult Services
- Work jointly with Special Education Needs Co-ordinators regarding funding matters in cases where joint funding is required
- Collaborate with Health services within the integrated Community Team for People with Learning Disabilities
- Attend meetings to establish holistic support plans with relevant stakeholders
- Promote service users' views and wellbeing in all interventions
- Raise and/or manage safeguarding concerns and address relevant matters
- Present cases at the weekly Risk Panel to request funding for new cases or amended care packages
Requirements:
- Post-qualifying experience as a Qualified Social Worker in the UK Social Care industry
- HCPC registration
- Enhanced DBS/CRB (acknowledged that Ackerman Pierce can provide)
- Eligibility to work in the UK
