Job Summary

The Senior System Discharge Coordinator will lead the delivery of a system-level discharge coordination centre for the Oxfordshire health and social care system on behalf of Oxford University Hospitals NHS FT, Oxford Health NHS FT, and Oxfordshire County Council.

Main Responsibilities
  • Lead the development and implementation of effective discharge pathways across the system.
  • Collaborate with system health and care leaders to coordinate care and support to reduce the number of days away from home for patients.
  • Manage the effective use of discharge pathways and lead system-wide change in practice and culture.
  • Provide operational leadership and management to teams across OUH, OH, and OCC to support effective discharges for the Oxfordshire system.
Key Performance Indicators
  • Delivery of an effective and multi-agency approach to transfer of care from a hospital setting to home.
  • Reduction in the overall use of bed provision within acute hospitals and community wards.
  • Contribution to a leadership culture of collaboration and trust.
  • Local compliance with national policies and guidance regarding hospital discharge.

Requirements
  • Good understanding of the drivers around hospital discharge and associated legislative frameworks.
  • Process and system improvement experience.
  • Previous experience of implementing a Transfer of Care hub or Discharge to Assess service is desirable.
