Nurse Case Manager - Complex Care

As Nurse Case Manager you will be seen as a wealth of knowledge and support for your team and be expected to have continuing responsibility for setting and monitoring standards and quality of care delivered to service users.

Due to the nature of our business and the role the successful candidate will require to be in possession of a full valid UK driving licence with access to their own vehicle.

Role: Nurse Case Manager - Complex Care
Where: Merseyside
Hours: Full-time hours, 35 per week.
Salary: Up to GBP38,000 depending on experience plus bonus.

What are we looking?

Somebody who is passionate through and through about delivering high quality care to our clients
Minimum of 1 years' experience of working in community care
A good understanding of the health and social care sector and the provision of person-centred care

What will your day-to-day look like?

Overseeing and managing staff within the care package.
To have continuing responsibility for setting and monitoring standards and quality of care delivered to service users.
To monitor and ensure high standard of record keeping in accordance with the NMC, national legislation and local standards enabling effective communication with other multi-professionals.
To take ownership and manage a caseload of complex clients and manage their care package both clinically and holistically.
To develop, review and implement service user care plans and risk assessments in line with CQC regulations and best practice guidance, and to assist in developing the skills of non-qualified staff to support you.
