*What you can add:*
\*You will provide a clinical pharmacy service to the gastroenterology ward on a rotational basis.
\*Support the provision and development of a holistic, specialist pharmaceutical care service to patients receiving high-risk, high-cost medication (e.g. advanced therapies, hepatitis anti-virals) delivered via Homecare services.
\*Support the services provided to outpatients and endoscopy and provide a clinical pharmacy service to the daycase unit.
\*Help provide a ward-based clinical pharmacy service according to Trust agreed standards including: individual prescription review to optimise therapy, confirmation of the patient’s medication history, advice on dosage, side-effects, cautions and monitoring required and offer duties.
\*Participate in ward rounds, clinical meetings e.g. ward user group or out-patient clinics as appropriate in order to provide pharmaceutical advice to prescribers and other health care professionals.
*What you’ll need:*
· _Qualified to Master’s Degree level (MPharm or equivalent)_
· _Member of General Pharmaceutical Council (GPhC)_
· _Post Graduate Diploma/MSc in Clinical Pharmacy or equivalent_
· _Clinical experience within gastroenterology/hepatology is Highly Desirable_
