Job summary

We have an exciting opportunity for a forward thinkingindividual, passionate about the development of excellence in palliative careand with substantial post-registration experience to have overallresponsibility for the clinical leadership and management of the hospiceclinical nurse specialist team.

You will drive the development and delivery ofthe service in keeping with the clinical strategy while ensuring high qualityevidence based specialist palliative care is delivered with care, compassionand dignity to patients and those that matter to them.

Main duties of the job

To work as a Head of thehospice community team, providing the clinical leadership, caseload managementand coordination necessary to ensure high quality, evidence based specialistand supportive palliative care is delivered with care, compassion and dignity, topatients and those that matter to them

Whilst leading an internal team ofHCNS Associates and support staff, work as a positive, pro-active role modeland autonomous practitioner demonstrating excellent standards of specialist assessment,care planning and evaluation of interventions to evidence the needs and choicesof people accessing hospice community services are met.

Job responsibilities

KEY PERFORMANCE AREAS

· Be an inspirational leader and exemplar role model whoclearly and consistently role models, supports and articulates the hospicevision, values and expectations regarding service quality to team members andnurtures their development to enable delivery of exceptional care

· Holding a reduced/time limited caseload and acting asnamed worker, undertake comprehensive specialist assessment of complexpalliative care needs and provide excellent clinical care

· Using clinical and analytical skills alongside ethicaland legal reasoning formulate, with the patient and their family,individualised recommendations/care plans and evaluate their effectiveness

· Effectively manage the care of patients with advanceddisease and complex needs who require specialist palliative care includingadvance care planning, symptom management and end of life care that meets theneeds of the individual and those important to them, promotes choice andempowers people to live life to the full and die with dignity in theirpreferred place of care

· Collaborate effectively with primary and communitycare colleagues and other partners to identify those individuals in the last 12months of life, promote awareness of and access to hospice services, encouragereferrals and reduce the fear associated with death and dying

· Work across departmental boundaries and with allrelevant professionals, groups and individuals to achieve awareness of hospiceservices, the community team model of provision, its outcomes and impact forpatients and those that matter to them

· Ensure effective systems are in place and maintainedto respond to referrals in a timely manner ensuring delegation takes account ofthe capacity and capability of team members to meet the needs of patients andthose important to them

· Develop, manage and maintain the service to be able torespond to patients and their families experiencing crisis situations withincommunity to prevent avoidable hospital admissions/readmissions negotiatingadmission to ELH inpatient beds where this can better meet patient need

· Lead on non-medical prescribing (NMP) within the team,including the development, implementation and review of NMP policy andprocedures

· Take a lead on specific projects and workcollaboratively with others making a significant contribution to the deliveryof hospice strategic objectives, governance agenda and local initiatives toimprove specialist and supportive palliative care across our community

· Develop and motivate team members to ensure thedelivery of high quality care and support that meets the standards set out inlegislation, national and local best practice guidance, clinical guidelines andhospice policies and procedures

· Manage the organisation, coordination and delivery ofclinical care ensuring patients, staff, volunteers and the public are supportedin ways that maximise health, safety, wellbeing and self-determination

· Function as a recognised specialist resource, withinand beyond the hospice, and ensure the community team provide professionaladvice and interventions in order to improve quality of life through palliationof symptoms and individualised end of life care to meet people’s wishes andneeds.

· Monitor the teams clinical caseloads, includingadmissions, episode closures and outcomes to provide assurance of a costeffective and equitable service that complements generic provision and isappropriate to the specific purpose, roles and skills of the hospice communityteam

· Lead on the development and provision of high qualityformal and informal education to all involved in the delivery of palliative andend of life care as identified by organisational training needs analysis

· Manage the service budget in line within agreeddepartmental limits, financial policies and procedures

· Be accountable and responsible for a defined portfoliodirected by service need which might include for example, clinical supervision,safeguarding.
