Job summary

CSH are currently looking for a Band 6/7 Physiotherapist or Occupational Therapist to join the UCR team. This is an exciting opportunity that will allow you to contribute to and participate in team objectives and service developments.

Main duties of the job

You will work as a member of the multi-disciplinary Urgent Community Response (UCR) team. Undertaking assessments, clinical diagnosis and treatment of patients including telephone triage, multi-factorial falls assessments, and patients who may require crisis management and/or rehabilitation interventions to prevent acute hospital admissions and to support complex hospital discharges. These patients often have complex presentations.

Job responsibilities

Principal duties to include:

Communication and Relationship skills

communicate effectively with all disciplines involved in a patients care, boththose from within the organisation and those outside, to ensure effectivemultidisciplinary working. This will include discussion of patient care, patientprogress and involvement in discharge planning.

To be able to communicate complex and sensitive information to patients, carerand other staff including imparting unwelcome news relating to their rehabilitationpotential and gaining consent for treatment.

To communicate any changes in care plan or treatment approach to otherrelevant members of the UCR team and the UCR Operational Lead asnecessary.

To ensure timely and effective communication with the Physiotherapy Team Leadand other multidisciplinary Team Leads on all professional matters.

To maintain accurate documentation in line with the Chartered Society ofPhysiotherapy standards and CSH Surrey standards and policies.

To attend staff meetings and actively participate ensuring understanding ofinformation imparted.

Assess patients understanding of proposed intervention. Gain valid consent andwork within a legal framework with patients who lack capacity to consent toassessment and or intervention.

To use a range of verbal and non-verbal communication skills to communicateeffectively with patients in order to progress rehabilitation and treatmentprogrammes. This may include patients who have difficulties in understanding orcommunicating patients who may be deaf, blind, have cognitive and orbehavioural problems, or who are unable to accept their diagnosis or presentingcondition.

To appreciate cultural differences and take these into account when agreeingtreatment plans with patients.

To manage patients who may be occasionally aggressive or confused.

To be able to manage potentially stressful, upsetting or emotional situations in anempathetic manner.

Analytical and assessment skills

To undertake comprehensive assessments of patients including those withdiverse or complex presentations or multi pathologies.

To use investigative and analytical skills to assess complex patients and supportother staff with their patients as required.

To devise individualised management and or treatment plans using clinicalreasoning skills.

Responsibilities for human resources including personal and people development

To work as part of the UCR team to provide an efficient and effective service over7 days.

To supervise junior UCR staff where appropriate.

To evaluate own and others practice and identify own and others developmentneeds.

To participate in the appraisal scheme as an appraisee and as an appraiser forjunior staff.

Participate in the supervision of students.

Deputise for the Physiotherapy Team Lead when required.

To provide training take a role in supervision and teaching of qualified andunqualified clinical staff to include nurses, occupational therapists andrehabilitation assistants.

To be responsible for and actively record your own personal development. Thismay include active participation in journal clubs, clinical reasoning sessions andchallenging patient sessions as participant or facilitator.

To ensure professional competence is maintained by participating in allContinuing Professional Development (CPD) opportunities and maintain aportfolio of evidence.
