JOB TITLE : PracticeNurse

REPORTS TO: PRACTICE MANAGER & GP PARTNERS

HOURS : approx 32hrs

Job Summary

Manage theday to day operations of the Practice in accordance with the Partnersinstructions and to ensure Practice compliance with Contractual duties andresponsibilities including GMS Contract, Quality and Outcomes Framework andEnhanced Services Schemes.

Treat,assist and direct patients in accessing the appropriate service or healthcareprofessional in a courteous, efficient and effective way. Provide generalassistance to the practice team and project a positive and friendly image topatients and other visitors, either in person or via the telephone. Duties mayvary at times under the direction of the Practice Manager, dependent on currentpractice workload and staffing levels. The post-holder will apply practicepolicies, standards and guidance. Participate in audit where appropriate.

Job responsibilities

Administration

To be involved in the delivery of managerial andadministrative aspects of the Quality and Outcome Framework (QOF) including thepreparation and submission of documents and evidence required for annual QOFreview purposes.

Ensure practice compliance with CQCrequirements.

To have a thorough knowledge of all Practiceprocedures.

To work in accordance of written protocols.

Prioritise workload to maximize efficiency

Practice Nurse duties

To provide all Practice Nurse duties, includingImmunisations, vaccinations, cervical smears, specialist clinics and generalevery day practice nurse procedures.

To be involved with the general organisation ofthe Practice ensuring that systems and procedures are in place to deliver highquality, responsive primary care services to patients registered with thepractice.

To be involved with and implement policies andprocedures ensuring coherence to nation standards and legislation.

Communicating with patients via telephone orother as would be expected at a GP practice.

Support Partners and other clinical staff in theimplementation and achievement of QOF clinical targets including regular reviewof Population Manager.

To be involved in and ensure that the Practiceis compliant with the requirements of the General Medical Services (GMS)contract and help ensure the practice meets the requirements of the PerformanceManagement Procedures.

Manage registers and implement recall systemsfor patients with Long Term Conditions.

Consulting with members of the practice team andoutside agencies.

Ordering supplies, Vaccines & medications.

Computer

Use of Emis Web software system.

Make adjustments to electronic patient records& update manual paper records as necessary.

Make referrals for patients to other agencies asrequired

Confidentiality

Ensure that you adhere to strict confidentialityguidelines at all times.

Be aware of anyone who may overhear yourconversation.

Ensure that any paperwork or onscreeninformation cannot be viewed by a patient or anyone not employed by thepractice.
