Job summary

Clinical Pharmacy Technician in General Practice is part of an excitingprogramme of transformation to develop a new model of care which addresses ourambition to deliver a person-centred co-ordinated care.

The Pharmacy Technician inGeneral Practice model is supported by the direction of national policyincluding the Five Year Forward View and GP Forward View where there is a needto better utilise the role of pharmacy within primary care to pro-actively helppatients stay safe and well and out of hospital as well as helping to reducethe demands on general practice

Main duties of the job

The role will include delivering effective medicines management and medicines optimisation, improving patient care and being part of a multidisciplinary team whilst working within the strategic objectives of Hillview Family Practice.

The post holder will help patients to get the best from their medicinesthrough effective medicines reconciliation after discharge and on othertransfers of care, medication use reviews, identification of patients suitablefor electronic repeat dispensing, promotion of online ordering, providingassurance of monitoring of high risk drugs, and handling of prescriptionqueries within the areas of competence.

Primary care experience would be an advantage however is not essential. You should hold a BTEC / NVQ level 3 qualification in pharmaceutical sciences and be registered with the General Pharmaceutical Council. If you have not already enrolled, you should be willing to enrol on the CPPE Primary Care Pharmacy Education Pathway.

Excellent communication, interpersonal skills and experience as a leader are essential plus being able to work as part of the practice team as well as autonomously.

Job description

Job responsibilities

Tosupport in the management of the repeat prescribing process, helping to managerepeat prescription requests, resolving queries where possible and producingprescriptions for clinical review and signature. To ensure patients haveappropriate monitoring tests in place when required.

•To undertake medicines reconciliation of hospital letters and dischargesummaries in line with local protocols and within competency levels.

Undertake medication use reviews Day to day handling of prescription queries within area of own competence Provide technical and administration support to clinical workforce and practice administration team (including prescription clerks)

•To promote and implement electronic repeat dispensing in appropriate patients;to promote online ordering of prescriptions.

•To work alongside the PCN clinical pharmacists to deliver certain aspects ofthe PCN Direct Enhanced Service (DES), structured medication reviews(SMRs).

•To undertake medication audits in priority areas, such as QOF, NICE guidance,and other agreed target areas, feeding your findings back to the network teamsand implementing actions as appropriate.

•Provide independent information to patients with regard to medicines andprescribing changes, initiating further support from other healthcareprofessionals where appropriate.

•To take a proactive approach in the safe handling of prescriptions.

Undertake training and development, to ensure the required knowledge and skills for practice-based work. Support self-care agenda through patient education on the importance of compliance and adherence to medication regimen and further provide holistic advice to manage patient health and well-being

Developand maintain key working relationships with:

•PCN Pharmacists

•Practice GPs, Practice Nurses and Healthcare Assistants

•Patients and Carers where appropriate

•Practice Management, Reception and other Administrative staff

•Community Nurses and other allied health professionals

•Community Pharmacists and support staff

Registration with the General Pharmaceutical Council NVQ level 3 in pharmacy services or equivalent Evidence of commitment to continuing professional development. Centre for Pharmacy Postgraduate Education (CPPE) primary care Pharmacy Education Pathway (or working towards or willing to enrol and work towards)
