North Halifax PCN is a well-established network of 4 practices who work collaboratively delivering care to our population of over 38,000 are are progressive PCN serving over 38,000 patients with embedded services, working in a hub-and-spoke model. Our multidisciplinary team comprises:

- 6 x Clinical Pharmacists

- 3 x First Contact Physiotherapists

- Podiatrist

- Paramedic

- Ageing Well Practitioner

- Phlebotomist

- Digital Transformation Lead

- Mental Health Practitioner

- Personalised Care Team comprising 3 Social Prescribing Link Workers and 3 Care Coordinators

The team is managed by a PCN Manager and clinically lead by the the Clinical Director, representing the PCN board. We have adopted the Lead Practice model of PCN, which allows us to offer continuous service for NHS pension.

Job responsibilities

Thefollowing describes the primary areas of responsibility for this role. Theclinical areas for delivery will be agreed after discussion with PCN leads andin line with the individuals own scope of practice and experience. This maychange according to the DES requirements and PCN/practice/patient needs.

Patient Facing/Telephone Structured Medication Review - Undertake clinical medication reviews with patients with multimorbidity and polypharmacy and order relevant monitoring tests according to practiceprotocols and policies. Review the on-going need for each medicine, reviewmonitoring needs and support patients with their medicines taking ensuring theyget the best use of their medicines (medicines optimisation).

Patients FacingMedicines Support - Provide patient facing clinics for those with questions,queries and concerns about their medicines in the practice.

Telephone Triage - Assist inmedication related triaging of patients as required by the practice.

Telephone Medicines Support - Provide a telephone helpline for patients with questions, queries and concerns about their medicineswith some bookable appointments.

Medicine Information to Practice Staff and Patients - Answer relevant medicine-related enquiries from GPs, other practice staff,other healthcare teams ( community pharmacy) and patients with queriesabout medicines. Suggest and recommend solutions. Provide follow upfor patients to monitor the effect of any changes.

Work with case managers, multidisciplinary (health and social care)review teams and hospital colleagues to manage medicinesrelated risk for readmission and patient harm. Put in place changes to reduce theprescribing of these medicines to high-risk patientgroups.

Enhanced Health in Care Homes In line with the DESlead/form part of the MDT working towards EHCH Care coordinator lead

Clinics As we expand the team, we are continuouslyenhancing our services to our partner practices by using Population Health Managementapproach, hypertension and diabetes management, lipid and other medicines optimisationprojects.

Management ofMedicines at Discharge from Hospital - To reconcile medicinesfollowing discharge from hospitals, including identifying and rectifyingunexplained changes. Perform a clinical medication review of medical records,produce a post-discharge medicines care plan including dose titration andbooking of follow up tests, and working with patients and community pharmaciststo ensure patients receive the medicines they need post-discharge. Set up andmanage systems to ensure continuity of medicines supply to highrisk groups of patients ( those with medicine compliance aids).

Signposting -Ensure that patients are referred to the appropriate healthcare professionalfor the appropriate level of care within an appropriate period of time pathology results, common/minor ailments, acute conditions, long term conditionreviews etc.

Repeat Prescribing- Adhere to the repeat prescribing protocols.

Medicines Quality Improvement Programmes - Identify and provide leadership on areas of prescribing and medicinesoptimisation. Conduct or work with other stakeholders on clinical audits andimprove projects. Present results and provide leadership on suggested changes.

InformationManagement - Undertake clinical audits of prescribing in areas directed by thePCN, feedback the results and implement changes in conjunction with thepractice's teams.

Medicines Safety - Adhereto national and local policyand guidance that affects patientsafety through the use of medicines, including MHRA alerts,product withdrawals and emergingevidence form clinical trials. Managethe process of implementing changes to medicines and guidance for practitionersand ensure compliance with medicines legislation.

Education andTraining - Provide education and training to primary healthcare team ontherapeutics and medicines optimisation. Provide training to visiting medical,nursing, pharmacy and other healthcare students where appropriate.

Care QualityCommission - Provide support to the general practice team to ensure thepractice is compliant with CQC standards where medicines are involved. Undertakerisk assessment and management and ensure compliance with medicineslegislation.
