Job summary

As part of our ongoing development of services we are seeking to recruit a Team Leader for our Patient Flow Hub. The Patient Flow Hub (PFH) is a new service launched by Pennine Care NHS Foundation Trust in 2019 as part of PCFT overall commitment to improving quality of care and supporting delivery of the ambitions in the Integrated Mental Health Programme to increase efficiency and support sustainability of the mental health systems. It is designed to improve the way in which patients are admitted to hospital and to ensure patients are discharged in a timely and appropriate manner.

The post holders' primary responsibility will be to lead the Patient Flow Hub in the areas of: admission to Hospital, transfers between wards, assisting and predicting flow through the in-patient pathway, assisting clinical teams with informal and formal delayed discharges, producing bed management related reports and analysis and oversight (with colleagues from other PCFT departments) of OAPs.

Main duties of the job

The post holder will be responsible for ensuring that the PCFT Trust values set out below are adhered to daily in their work, in their team and whilst providing services to patients and their families. o Compassionate o Accountable o Responsible o Effective o Safe

Responsible for identifying potential complex discharges on admission, and in collaboration with the multidisciplinary team set and monitor discharge dates to assist the service in ensuring robust capacity and flow.

The post holder will be responsible for tracking and leading on complex discharges to prevent delayed transfers of care.

To work in collaboration with a dedicated multidisciplinary team, Local authority and other relevant personnel in both the hospital and community to plan and manage timely and appropriate transfers of care (discharge) for patients from hospital.

The post holder will be the main source in redirection of admissions in accordance with assessment of need through the identification of the appropriate care setting such as PICU or forensic services

Essential

RMN or equivalent (SW/ OT/AHP) Extensive experience of working in acute mental health services. Educated to degree level Post qualification experience to post graduate diploma level

Desirable

Experience of working with Local Authority, CCG, other mental health trusts, Police and other relevant stakeholders within the specified field (patient flow)

Sound understanding of current government policies in relation to Acute Care including National Guidance, MHA, Risk Assessment, Risk Management processes and CPA processes Sound understanding of the Trust's Acute Service model Good working knowledge of assessment, care planning, implementation and evaluation processes Excellent understanding of the principles of capacity management and bed management procedures.

Desirable

Knowledge of psychosocial assessment tools and brief psychosocial interventions. Knowledge of a range of management approaches.

Knowledge

Experience of MDT working Evidence of relevant experience working in an acute mental health or specialist service setting

Experience of independently managing a case load.
