We are a CQC Good GP practice, with a list size of approx. 7300 patients in the Manor area of Sheffield. We are a friendly team, committed to the ongoing development of all our staff. We are proud to be a training practice to GP Registrars, F2's and medical students.

The successful applicant will share our enthusiasm for education and will help deliver a high level of patient care, becoming a key member of staff. Our team consists of 4 GP Partners, salaried and retainer GPs, ANP, Paramedics, 3 PN's, HCAs and a Pharmacist along with a well-established supportive reception/admin team. We are a proactive member of the GPA1 network and the Deep End group of practices.

Job responsibilities

CLINICAL:

Complex wound management: providing diagnosis, management plan and treatments. Initiate and carry out programmes of health screening and health promotion Vaccinations and immunisations in line with individual prescription, and Patient Group Directions. Adult and childhood immunisation service. Travel health advice: offer a holistic approach to travel health including comprehensive advice for patients prior to travel. Aural Care, working to protocols Infection control: support patients and colleagues in adopting sound infection control measures, applying infection control measures within the practice according to local and national guidelines. Emergency management competent in anaphylaxis and resuscitation techniques Mental health: recognise psychological needs of patients. Male health; initiate opportunistic consultations with patients to cover issues of mens health, making referrals as appropriate. Family planning and womens health: provide a comprehensive family planning service and sexual health service to patients Provide chronic disease management programmes including but not limited to asthma, COPD, diabetes, CHD, hypertension and anticoagulation Develop practice protocols for the surgery and clinical protocols for the clinical team to work within

ADMINISTRATIVE:

Plan, implement and maintain nursing team appointment and recall systems in conjunction with administrative staff Liaise with clinical colleagues to provide nursing team cover for the practice Involved in implementation of standard setting and audit

EDUCATIONAL:

Advise and support other members of the primary health care team Update Nurse Colleagues, GPs and Practice Manager following own educational activities. Share newly acquired skills and knowledge

The post holder may be required, from time to time, to undertake any other tasks necessary to ensure good patient care or the smooth running of the Practice; these tasks would fall within what may reasonably be considered to be commensurate with the post of Practice Nurse.

Person Specification

Experience

Essential

Broad knowledge of clinical governance. Knowledge of Infection Prevention Control. Understands the requirement for PGDs, PSDs and associated policy

Desirable

Experience of working in Primary Care or a GP Practice. Interest and basic knowledge in long-term conditions. Knowledge of LES/DES/QoF/CQC related clinical and administrative work.
