Job Title: Practice Nurse

Job Summary:

We are seeking a highly skilled and experienced Practice Nurse to join our team at Primary Care Sheffield. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of excellent healthcare services.

Main Responsibilities:
  • Provide comprehensive patient care, including screening, clinical assessment, diagnosis, treatment, and follow-up care.
  • Manage chronic diseases, such as asthma, COPD, and diabetes, and provide health promotion and education to patients.
  • Work collaboratively with the clinical team, including GPs and other healthcare professionals, to ensure seamless patient care.
  • Undertake routine tasks, such as phlebotomy, injections, and wound care, and provide a comprehensive family planning service and sexual health service.
  • Participate in clinical governance and contribute to the development and maintenance of an excellent learning environment.
Requirements:
  • Minimum of 2 years post-registration experience in Primary Care.
  • Experience in managing long-term conditions, health promotion, and education.
  • Knowledge of national standards, policy developments, and evidence-based practice.
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to work under pressure, prioritize workload, and manage patient risk effectively.
Person Specification:
  • Registered General Nurse (currently registered with the Nursing & Midwifery Council, with a valid PIN and no restrictions).
  • Experience in cervical cytology, vaccine and immunization training, and home visits.
  • Ability to establish and maintain effective communication pathways within Primary Care and with key external stakeholders.
