We are seeking a highly skilled and experienced Practice Nurse to join our team at Primary Care Sheffield. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of primary care services within our GP-led organisation.
Main Responsibilities:- Provide comprehensive patient care, including screening, clinical assessment, diagnosis, treatment, and follow-up care.
- Manage chronic diseases, such as asthma, COPD, and diabetes, and provide health promotion and education to patients.
- Undertake cervical cytology, spirometry, and other diagnostic tests as required.
- Administer injections and provide wound care using aseptic technique.
- Communicate effectively with patients, families, and healthcare professionals to ensure delivery of a coordinated service.
- Minimum of 2 years post-registration experience in primary care.
- Experience in managing long-term conditions, health promotion, and education.
- Knowledge of national standards, policy developments, and evidence-based practice.
- Excellent interpersonal, verbal, and written communication skills.
- Ability to work autonomously and as part of a team.
- A dynamic and supportive work environment.
