Community Palliative Response Senior Nurse Paramedic Overnight

Job summary
Rennie Grove Peace Hospice Care services are provided 24 hours a day, 7 days a week.
The post holder will be required to work across shifts covering 9:15pm 7:15am as part of an extended community team working in the patients home environment including, as required, in care homes.
Although based in the community, post holders may occasionally be required to work in other Rennie Grove Peace care settings in order to ensure adequate staffing levels and to gain experience of the wider hospice delivery of care.
Clsoing date is 05 December 2024 at midday.
Main duties of the job
You will need to demonstrate your abilities in the following areas:
Proactively contribute to the delivery of high-quality safe evidence based care.
Responding to SOS calls, visiting patients in their own homes t provide palliative and endof life care.
To lead on and co-ordinate assessment, planning and evaluation of individualised patientcentred care.
Ensuring a holistic approach that is sensitive and responsive to the patientsever changing needs.
To be responsible for the co-ordination and smooth running of a shift and visits ensuringit is well organised.
As a member of the wider Rennie Grove Peace Care staff sign up and demonstratedelivery of the organisational agreed mission, vision and values.
Acting as a Rennie Grove Peace ambassador within the community.
As a lone worker, be responsible for your own and others safety.

Job description
Job responsibilities
To undertake holistic needs assessment that explores individual patients and theircarers/families physical, emotional spiritual, practical, welfare right and if appropriatethe families pre and post bereavement need, using a variety of tools to support theprocess.
To be able to develop, update and monitor the individualised care plans ensuring theyare patient centred and support the identified patient needs, including ongoing referralsto other services as required.
To be able to deliver both generalist basic and complex specialist nursing careinterventions that are responsive to the individual patients requirements.
To be able to communicate highly sensitive information, ensuring patients and theirfamilies/carers are adequately supported. To be able to demonstrate through electronic and paper documentation and verbalcommunication the care undertaken and the patient outcomes.
To be responsible in leading on initial holistic assessment and reviews of care whenrequired and discharge planning from service.
To be able to communicate effectively to own team, extended team and outside agencies,ensuring continuity of care is maintained across the different care settings/home.
To follow all agreed clinical procedures and statutory regulations related to MedicineManagement.
At identified shifts be responsible for dealing with any service enquiries, calls andreferrals as required.
To signpost and inform patients and their families/carers of the range of servicesavailable to them, external resources and day to day information relating to the service.
Attend relevant MDT meetings as appropriate.
Person Specification
Experience
Essential
Registered General Nurse/Paramedic. Relevant postgraduate qualification relating to speciality or evidence of transferable experience. Significant post registration palliative, oncology or long-term conditions experience. Able to communicate effectively both verbally and in writing. Good administrative and record keeping skills and able to work with good attention to detail. Strong organisational skills. Understanding of current palliative care matters. IT literate and experience of using an electronic patient record system. Ability to deal with complex and difficult emotional situations. Ability to work autonomously and as part of a team. Strong problem-solving skills and able to implement new initiatives. Willingness to undertake a nominated extended role (champion/mentor) Calm and approachable manner. Flexible attitude to working Willingness to work in other areas of patient services. A commitment to the visions, values and objectives of Rennie Grove Peace Hospice.
Desirable
Mentorship course or willingness to undertake mentorship responsibility. Experience of participating in clinical audit/quality management-
