Community Prt Senior Nurse/paramedic

**The main duties of the job involve**:
- Proactively contribute to the delivery of high quality safe evidence based care
- Responding to SOS Calls visiting aptients in their own home to provide palliative and end of life care.
- To lead on and coordinate assessment, planning and evaluation of individualised patient centred care. Ensuring a holistic approach that is sensitive and responsive to the patients ever changing needs.
- To be responsible for the coordination and smooth running of a shift and visits ensuring it is well organised.
- As a member of the wider Rennie Grove Peace Hospice Care staff sign up and demonstrate delivery of the organisational agreed mission, vision and values.
- As a lone worker be responsible for your own and others safety by adhereing to the Lone Worker policy and precedure requirements.
