Main duties of the job

To perform as an autonomous general practitioner working independently with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. Provide expert professional advice to patients, carers and colleagues whilst maintaining clinical excellence. To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct, ensuring the safety of patients and quality of their care is primary.

Meir PCN is a membership comprising of six practices - Adderley Green Surgery, Meir Medical Practice, Meir Park & Weston Coyney Medical Practice, Dr Miles & Valasapalli, Trinity Medical Practice and Willow Bank Surgery - covering the Meir, Longton & Blythe Bridge areas of Stoke on Trent.

We currently have a PCN population of approximately 37,500 patients and the PCN staff work in conjunction with the local general practices, local services and other organisations to support both local and national priorities.

Job responsibilities

Main Responsibilities

Clinical Duties:

Provide face-to-face and telephone/videoconsultations for routine and urgent care.

Conduct patient home visits / care homevisits

Manage patient care in surgery, includinginvestigating and reviewing results, developing care plans, and makingreferrals to secondary care or other providers.

Issue and sign repeat prescriptions, ensuringthey align with national/local guidelines.

Administer and handle patient-relatedqueries, test results, referral letters, and other tasks in a timely manner.

Record clear, contemporaneous consultationnotes using EMIS clinical system.

Ensure safe, evidence-based, andcost-effective care, in line with Practice disease management protocols andNICE guidance.

Independently prescribe medications withinyour scope of practice.

Review and manage patient medicationsfollowing appropriate policies and local clinical guidelines.

Utilise other available clinical and supportstaff in delivering a holistic and patient centred care.

Actively participate in MDT and careconference meetings, as required.

Health Promotion and Patient Support :

Regularly screen for lifestyle factors andsupport patients in adopting health promotion strategies and self-careprinciples.

Contribute to the delivery of anticipatorycare plans.

Understanding of evidence-based practice. Evidence of working autonomously, with some support and supervision Ability to effectively triage, assess and diagnose Requesting pathology tests and processing the results, advising patients accordingly Assessing and managing acute deteriorations/exacerbations of Long-Term Conditions Understands the importance of evidence-based practice Ability to assess and manage patient risk effectively and safely. Broad knowledge of clinical governance Ability to work within own scope of practice Excellent communication skills (written and oral) including recording of accurate clinical notes.
