**Job Summary**

To support our PCN practices in the delivery of the Facilitation of Admission Avoidance (FAAS) service, delivering care within their scope of practice to the entitled patient population. The care coordinator will deliver and assist clinical staff in preventative care, health promotion and patient education.

Contracted hours are negotiable and may be worked across the core hours of 8am to 6pm, Monday to Friday.

**Primary Responsibilities**

The following are the core responsibilities of the care coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

- Visit patients in their homes to conduct holistic needs assessments - minimum 6 visits per day
- Conduct holistic needs assessments in the surgeries
- Hand out patient satisfaction surveys after each consultation and assist patients with completion of these if needed
- Complete referrals for patients when required
- Liaising with surgery staff regarding patient concerns
- Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately
- Facilitate routine and 24-hour BP monitoring, advising patients accordingly
- Carry out BMI checks as directed
- To possibly, when trained, undertake venepuncture
- Ensure specimens are recorded and ready for onward transportation
- Ensure clinical waste is removed from clinical areas and sharps bins replaced in accordance with the practice IPC policy
- Deliver opportunistic health promotion where appropriate
- Ensuring samples are stored in appropriate containers
- Accurate, legible labelling of all samples to confirm patient identity
- Checking patient identification against GP/nurse request and confirming with patient whenever possible
- Ensuring that anomalies are reported to a senior member of staff
- Notifying the GP/nurse of any significant patient issues
- Liaising with and assisting members of the administrative team

**Secondary Responsibilities**

In addition to the primary responsibilities, you may be requested to:

- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the PCN

Healthcare qualification (level 3 or 4) or working towards gaining equivalent level Phlebotomy certification
