Clacton PCN presents an exceptional opportunity for a skilled nurse to become part of our welcoming and patient-centered team on a full-time basis. This role entails working across five days a week, including some weekends as mutually agreed. The position is offered as a fixed-term contract.
Main Responsibilities- Conduct consultations, gather patient histories, perform physical assessments, and interpret clinical observations while identifying critical signs that necessitate escalation of care.
- Exhibit safe and proficient clinical decision-making in alignment with established policies, procedures, and guidelines.
- Engage in effective communication and collaboration with general practice, the broader PCN team, and other healthcare providers.
- Maintain professionalism in accordance with regulatory standards, ensuring confidentiality and accurate record-keeping.
- Lead initiatives aimed at enhancing cancer screening rates, improving patient experiences throughout the cancer care pathway, and facilitating advanced care planning.
- Collaborate with member practices to identify community health needs, fostering strong relationships to support patients and enhance care delivery.
The ultimate goal of this position is to elevate cancer care within our community, providing a remarkable chance to positively impact patient outcomes as part of a progressive and supportive PCN.
About the CandidateApplicants should possess the following qualifications and experience:
- A relevant nursing diploma or degree.
- Current NMC registration with an active PIN.
- A minimum of three years of post-registration experience or equivalent.
- Demonstrated understanding of personalized care approaches.
- Evidence of advanced clinical skills and knowledge.
- Experience managing patients with complex health needs.
- Awareness of public health issues pertinent to the local population.
- Familiarity with the Primary Care Network Direct Enhanced Service and local healthcare visions.
- Strong understanding of current challenges within the NHS.
- Professional pride and the ability to manage time effectively.
- Excellent communication skills and the capacity to handle high-pressure clinical situations.
- Willingness to adapt and work flexibly to deliver effective healthcare.
- Demonstrated clinical knowledge of various long-term and chronic conditions.
- Understanding of the Quality Outcome and Investment Framework.
- A commitment to continuous learning and professional development.
