Job Summary

Old Road Medical Practice is seeking a skilled Nurse Prescriber to contribute to our multidisciplinary team. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and promoting health and well-being.

Main Responsibilities
  • Conduct comprehensive health assessments, administer medications, and perform diagnostic tests to ensure accurate diagnoses and effective treatment plans.
  • Develop and implement individualized health interventions to address patients' complex needs, promoting optimal health outcomes and quality of life.
  • Collaborate with patients to create personalized care plans, focusing on prevention, education, and empowerment.
  • Manage and treat long-term conditions, adhering to national and local guidelines, and practice protocols.
  • Review and optimize medication regimens, ensuring therapeutic effectiveness and patient adherence.
  • Respond to urgent or emergency situations, prioritizing patient needs and providing effective care.
  • Support patients with sexual health and contraception needs, as well as cervical cytology consultations.
  • Implement and participate in vaccination and immunization programs for adults and children, including travel-related vaccinations.
  • Provide wound care services, addressing the needs of patients with complex wounds.
  • Lead asthma, COPD, and diabetes clinics and reviews, ensuring comprehensive care and optimal patient outcomes.
  • Conduct new patient and health checks, identifying potential health risks and developing targeted interventions.

  • Essential:
    • Proven clinical skills and experience in treatment room consultations, phlebotomy, ECG recording, screening, and management of long-term conditions.

    • Knowledge and experience in supporting patients with long-term conditions.
    • Understanding of current issues in nursing and the wider NHS.
  • Desirable:
    • Independent Nurse Prescriber qualification.
