About the Role
Primary Care Careers is seeking a skilled Nurse Prescriber to join our team in a dynamic and rewarding role. As a Nurse Prescriber, you will play a vital part in delivering high-quality patient care and contributing to the success of our primary care services.
Key Responsibilities
- Undertake comprehensive health assessments, administer medications and treatments, and perform diagnostic tests to ensure accurate diagnoses and effective patient care.
- Develop and implement individualized health plans to meet the unique needs of patients with long-term conditions, promoting health and well-being through evidence-based practice.
- Manage and treat long-term conditions in accordance with national and local policies, working collaboratively with patients to optimize treatment outcomes.
- Review medication regimens to ensure therapeutic effectiveness, supporting patients in adhering to prescribed treatments and promoting medication safety.
- Prioritize health problems and intervene appropriately in urgent or emergency situations, providing effective emergency care and supporting patients in complex situations.
- Support and manage the health needs of women presenting for sexual health and contraception services, as well as cervical cytology consultations.
- Implement and participate in vaccination and immunization programs for adults and children, including patients traveling abroad.
- Meet the needs of patients presenting for opportunistic wound care, promoting wound healing and preventing complications.
- Hypertension review and management, including blood pressure monitoring and medication optimization.
- Run asthma, COPD, and diabetes clinics and reviews, providing comprehensive care and education to patients with these conditions.
- New patient and health checks, ensuring timely and effective identification of health issues.

Person Specification
- Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
- Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work, valuing the dignity of patients, their relatives, and carers, as well as colleagues at the practice.
- Knowledge and experience of supporting long-term conditions, including asthma, COPD, and diabetes.
- Insight and understanding of current issues in nursing and the wider healthcare sector.
Qualifications and Experience
- A relevant nursing degree, such as a Bachelor of Science in Nursing or a Master of Science in Nursing.
- Current registration with the Nursing and Midwifery Council (NMC) and an active PIN.
- Independent Nurse Prescriber qualification, enabling you to prescribe medications and treatments.
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
- Competence to undertake asthma, COPD, and diabetic clinics and reviews, holding relevant certificates.
