Old Road Medical Practice is seeking a skilled and compassionate Nurse Prescriber to join our multi-disciplinary team. As a key member of our primary care team, you will play a vital role in providing high-quality patient care and contributing to the delivery of our practice's vision.
Key Responsibilities
- Undertake health assessments, administer medication and treatments, and perform diagnostic tests to ensure patients receive the best possible care.
- Provide and evaluate a range of health interventions to meet patients' health and social care needs.
- Develop and implement individualized care plans to promote health and well-being, and manage long-term conditions in line with national and local policies.
- Review medication for therapeutic effectiveness and work with patients to support adherence to prescribed treatments.
- Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
