Shelton & Hanley PCN are a group of practices working together to focus on local patient care. We are a small multi-disciplinary team, covering 3 practices across Shelton, Hanley and Birches Head covering a total population of around 38,000.

We have a strong leadership, led by our PCN Clinical Director and a supportive management team.

We are looking to expand our multi-disciplinary team which currently includes clinical pharmacists, pharmacy technicians, care co-ordinators, physician associates, a health and wellbeing coach and a mental health practitioner.

We are a very forward thinking and innovative PCN who utilises to the full, the skills and experience of our team members. We are supportive of professional development and pride ourselves on supporting peer support for these new roles whilst working in a motivating and friendly environment.

We work collaboratively with our local Federation who support core General Practice and sustaining the future for General Practice. The Federation are facilitating and supporting the recruitment of the new roles within our Primary Care Network.

**Job Overview**

Social prescribing link workers help patients to improve their health, wellbeing and social welfare by connecting them to community services. This can include activities such as art, gardening, leisure activities and singing classes. You may help them seek more practical advice, for example on debt or housing issues. A Social prescriber gives people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing.
You will work closely with our GPs to advise on treatment and healthcare for patients, to include those presenting in the primary care setting and those in the community. This will involve meeting with patients in practice, in the community, and in their own homes.
You will work collaboratively with our multi-disciplinary team of pharmacy, mental health and care coordination to best meet the needs of our patients.
This is a new and exciting role for you to develop and strive to support the needs of the PCN going forward.

as members of the PCN’s team of health professionals, take referrals from the GP Practices and from a wide range of agencies to support the health and wellbeing of patients;
assess how far a patient’s health and wellbeing needs can be met by services and other opportunities available in the community;
co-produce a simple personalised care and support plan to address the patient’s health and wellbeing needs by introducing or reconnecting people to community groups and statutory services, including weight management support and signposting where appropriate and it matters to the person;
evaluate how far the actions in the care and support plan are meeting the patient’s health and wellbeing needs
provide personalised support to patients, their families and carers to take control of their health and wellbeing, live independently, improve their health outcomes and maintain a healthy lifestyle;
develop trusting relationships by giving people time and focus on ‘what matters to them’;
take a holistic approach, based on the patient’s priorities and the wider determinants of health, including supporting people to take up employment, training and welfare support;
explore and support access to a personal health budget where appropriate;
manage and prioritise their own caseload, in accordance with the health and wellbeing needs of their population; and
where required and as appropriate, refer patients back to other health professionals within the PCN.

A strong awareness of health and social care need.
Experience of working in a community development context, health and social care, learning support or public health / health improvement.
Commitment to professional and personal development.
Able to work from an asset-based approach, building on current community and personal assets.
Good IT and keyboard skills.
