Job summary

An exciting opportunity has opened for an Advanced Nurse Practitioner to work within our Practice, Bowland Medical Practice in Wythenshawe, South Manchester.

We are looking for an experienced nurse acting within professional boundariesto provide care for patients from initial history taking, clinical assessment,diagnosis, treatment and evaluation of care.

The post holder will also undertake general treatment room dutiesas demand dictates and participate in chronic disease management.

They will be responsible for same day and planned patient care needs, minor or complex, in primary care.

To support the practice to ensure compliance with its statutory andregulatory obligations, such as but not limited to the Care Quality Commissionand Data Protection Act 2018(GDPR)

Main duties of the job

Assess, diagnose and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team.

Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations to the team so that referrals can be appropriate.

Assist in telephone triage of urgent cases.

Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.

Undertake Learning Disabilities reviews both in the practice and when home visiting.

Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.

Perform investigatory procedures requested by the GPs.

Visit housebound patients as agreed with their GP or the on-call team.

Always Maintaining confidentiality of patients records.

Makes effective use of the IT system EMIS by ensuring the consistent and accurate entering of data and codes, ensuring all contacts with patients are legibly recorded.

Contributes to practice/team audit /research as required.

Support the equality, diversity and rights of patients, carers and colleagues.

Follow practice infection control procedures, promote and maintain appropriate health & safety standards in line with the practice Health & Safety Policy.

Qualifications

Essential

Registered general nurse Prescribing Qualification Higher study in clinical practice and Advanced Level Clinical Assessment Reasoning and Decision Making

Desirable

Diabetes, asthma, CHD, COPD diploma or equivalent

Experience

Essential

Proven employment history which supports the application of sound clinical knowledge. Up to date portfolio demonstrating continuous professional development and learning.

Desirable

Primary Care or Urgent Care Environment experience (including Minor Illnesses) Basic Life Support certification from the Resuscitation Council or equivalent within last 12 months.
