Main duties of the job
We are looking for a District Nurse who possess experience of working within the community setting, you will have a broad set of clinical skills and the ability to provide supportive leadership and operational management to the wider community nurse team. This role will require you to have overall responsibility for the caseloads within Northwich Firdale DN Team to ensure that high quality, efficient and safe care is provided.
You will report into the respective Community Operational Manager who will support and guide you through regular 1-1 meetings. This role will also give you experience of working with our partners within the wider Integrated Care System.
We are looking for caring, understanding, enthusiastic and flexible leader who possesses the clinical, leadership and digital skills to promote quality improvement for a local population.
In return you will benefit from a structured and robust induction package and the development of competencies within a very supportive team. You will also have an allocated mentor to work alongside you during the induction and probation process.
About us
Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.
We employ almost 5,000 members of staff, provide around 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health.
The Trust is also part of Central Cheshire Integrated Care Partnership (CCICP), a unique local health partnership that provides a range of community services for people across South Cheshire and Vale Royal.
During the Coronavirus pandemic, the Trust was recognised nationally for its 'Be Safe Be EquiPPEd' campaign, which aimed to make the Trust as safe as possible for staff and patients, through supporting the correct use of PPE.

To lead others from a base of clinical credibility in order to model and facilitate improved practice and partnerships. To demonstrate positive leadership skills identifying and prioritising the workload to ensure a safe operational system, referring to Clinical Services Manager as necessary. To make a proactive contribution to the future development of the community nursing service. To support the Clinical Services Manager in the effective management of financial budgets. To respond promptly to any incident or risk relating to patients, self or staff within the team which occur whilst on duty and ensure the timely and accurate reporting of the incident/risk in line with Trust policy. This includes the investigation of all incidents and formulation of action plans to ensure that identified issues/risks are addressed. To monitor and manage team members sickness in line with Trust absence management policy, and record on the recognised Trust system. Liaise with DN Team leaders within a Locality, to co-ordinate cover for colleagues caseloads in their absence. To manage all annual leave requirements within the team, ensuring service delivery is maintained throughout the year. Under guidance from the Clinical Service Manager lead on the recruitment, selection and induction of new staff. Work with the Clinical Service Manager to ensure efficient and effective use of the workforce, implementing skill mix and staff deployment changes as appropriate to meet the changing needs/demands of patients. Be responsible for ensuring all team members are up to date with statutory and clinical training, keeping an accurate training record for the team Manage the day to day on going delivery and development of the service ensuring delivery of a service that is responsive, high quality, efficient which meets contractual obligations. To have responsibility for appraisals/ PDP within the team in line with Trust policy and ensure the training and development needs of staff are identified including Statutory and Mandatory training To support the Clinical Service Manager in ensuring the high performance of the team and individuals within it to address any capability issues, in line with Trust policy, To be responsible for the cascading of information relevant to team members and the implementation and chairing of regular team meetings. Ensure that all team members have opportunity to receive clinical supervision and to provide regular mentorship and coaching to staff across the community nursing teams. Assess, plan and implement a programme of care to meet the patients physical, social, emotional and environmental needs evaluating care at regular intervals, encompassing a rehabilitative/self-care approach where necessary. To facilitate attendance and representation at multi-disciplinary meetings in order to work in collaboration with the wider primary health care team, acting as case manager/care coordinator where appropriate Promote clinical excellence by ensuring an evidenced based approach to care complying with the agreed Trust policies and procedures and maintaining own clinical competencies within a clinical leadership role. To participate in the planning and implementation of complex care packages in the community, liaising with multi agency professionals to explore all options and care pathways. To lead the team in the management of long term conditions, end of life care and specialist nursing skills including leg assessment and management for patients identified within in the service specification. To lead the team in raising awareness of policies and procedures of the Trust and ensuring compliance in line with scope of professional practice. To work in partnership with General Practitioners, Primary Health Care team members and other stakeholders, liaising with Clinical Service Manager as appropriate. To have knowledge of and be able to effectively use/refer to local services and resources to contribute to the delivery of effective patient care as appropriate. To influence, support and take an active role in Public Health/ Self-care of the individual and community. Work with the Clinical Service Manager to ensure that staff are working to the competency framework in order to identify personal and professional capability to aid with succession planning. To engage in and influence professional meetings and case conferences to ensure appropriate attendance and dissemination of information as deemed necessary. Participate fully in the Nurse Prescribing initiative ensuring assessment of prescribing need is evidence based and cost effective and adheres to Trust prescribing formulary. To refer and participate in the decision making process in cases of Safeguarding and liaise with other disciplines and agencies as appropriate, in line with Trust policy. Accurately record and maintain all patient contacts in line with Trust and NMC policy.GOVERNANCE
