Job Summary
We are seeking a highly skilled and experienced District Nurse/Case Manager to join our team at Mid Cheshire Hospitals NHS Foundation Trust. As a key member of our Integrated Care Community Team, you will be responsible for leading the delivery of person-centred, safe, and effective care to patients in their own homes and community.
Key Responsibilities
- Manage and coordinate patients requiring community nursing input, including assessment of complex nursing care needs and development of individualized care plans.
- Oversee the implementation and evaluation of programmes of care, ensuring high-quality, evidence-based practice.
- Work collaboratively with the integrated community health care team, across professional and organizational boundaries, to provide 24-hour service delivery.
- Provide clinical leadership to the community nursing team, supporting the district nurse team leader in leading the team to deliver high-quality care.
Requirements
- Extensive experience in community nursing, with sound clinical knowledge and skills in managing patients with complex needs, long-term conditions, and end-of-life care.
- Passion for quality patient care and commitment to ongoing clinical professional development.
- Ability to develop leadership, organizational, and communication skills, and manage challenging situations in a professional and flexible manner.
- Travel is an essential part of this role, requiring flexibility and adaptability.
