About the Role

We are seeking a highly skilled and experienced District Nurse / Case Manager to join our team at Mid Cheshire Hospitals NHS Foundation Trust. As a key member of our community nursing team, you will be responsible for managing and coordinating patients requiring community nursing input.

Key Responsibilities

  • Assess and develop individualised care plans for patients with complex nursing needs
  • Lead and manage a caseload of patients, providing high-quality, evidence-based care
  • Work collaboratively with the community health care team to ensure seamless service delivery
  • Provide clinical leadership and support to the community nursing team

Requirements

  • District nurse qualification and over 2 years' experience in community nursing
  • Excellent interpersonal and communication skills, written and verbal
  • Experience in leading and managing staff and caseload management
