About the Role:The successful candidate will have a strong background in community nursing, with experience in assessing and managing complex nursing care needs. You will be responsible for developing and implementing individualized care plans, working closely with patients, families, and other healthcare professionals to ensure seamless care delivery.
You will also have the opportunity to work as part of an integrated community healthcare team, collaborating with professionals from various disciplines to provide comprehensive care to patients. As a District Nurse, you will be expected to demonstrate strong leadership and management skills, with the ability to deputize for the team leader as needed.
Key Responsibilities:- Assess and manage complex nursing care needs for patients in their own homes and communities
- Develop and implement individualized care plans, working closely with patients, families, and other healthcare professionals
- Provide high-quality, patient-centered care, ensuring seamless care delivery
- Work as part of an integrated community healthcare team, collaborating with professionals from various disciplines
- Deputize for the team leader as needed, demonstrating strong leadership and management skills
