About the Role
The Mid Cheshire Hospitals NHS Foundation Trust is seeking a highly skilled and experienced District Nurse to join our team as a Community Health Manager. This is an exciting opportunity to work in a dynamic and patient-centered environment, providing high-quality care to individuals in their own homes and communities.
Key Responsibilities
- Manage and coordinate patients requiring community nursing input, ensuring timely and effective care delivery.
- Oversee the assessment of complex nursing care needs, developing and implementing individualized care plans.
- Deputize for the team leader as required, ensuring continued efficient and effective service delivery.
- Work collaboratively with the integrated community healthcare team, promoting seamless care transitions and coordination.
- Provide clinical leadership to the community nursing team, driving high-quality, evidence-based care practices.
Requirements
- District nurse qualification and over 2 years of experience in community nursing.
- Excellent interpersonal and communication skills, with the ability to lead and manage staff.
- Experience in caseload management, with a focus on patient-centered care.
- Desirable: V150 or V100, mentorship, and experience in end-of-life care.
