To work as a member of the multidisciplinary care team, providing and managing holistic care to people who have life limiting conditions.

Main duties of the job

Responsible for assessing and managing the needs of patient assessing, planning, implementing, and evaluating the care, followingclearly defined policies, protocols, procedures, or codes of conduct.

This position involves 3 x 13 hour shifts per week, and these shifts include days, nights, weekends, and bank holidays.
