Job overview

The post holder is responsible, as part of a multidisciplinary clinical team, for delivering specialist nursing care. The post holder will focus on the open access follow up care of patients treated for breast cancer as well as supporting patients with breast cancer at all stages of their pathway. This will be achieved through the application of specialist knowledge, skills and 
practice. 

Main duties of the job

To develop, implement and monitor in conjunction with the advanced nurse practitioner and other members of the MD team where necessary, the open access follow up of patients treated for breast cancer. Provide clinical leadership and a key worker for people diagnosed with breast cancer and being followed up on the open access pathway and ensure that each person receives a high standard of personalized care consistent with the Macmillan Right by You Principles.  Provide every person diagnosed with breast cancer and followed up on the open access pathway with supportive conversations at key times in their journey, particularly at the time of diagnosis and during their open access follow up as required. Provide every person diagnosed with breast cancer with a holistic needs assessment at the time of diagnosis, at the end of treatment and at any time during their open access follow up when their condition or situation requires it Provide every person diagnosed with breast cancer a personalised care plan at the points in their pathway previously mentioned. Provide treatment summaries and health and well being interventions to all patients. Provide the appropriate information working closely with the Macmillan
