Job Overview

The post holder will provide specialist nursing care to patients treated for breast cancer, focusing on open-access follow-up and supporting patients at all stages of their pathway. This role requires the application of specialist knowledge, skills, and practice.

Main Responsibilities
  • To develop and implement the open-access follow-up of patients treated for breast cancer, working collaboratively with the advanced nurse practitioner and multidisciplinary team as needed.
  • Provide clinical leadership and key worker support for patients diagnosed with breast cancer and being followed up on the open-access pathway, ensuring personalized care consistent with the Macmillan Right by You Principles.
  • Deliver supportive conversations at key times in the patient journey, particularly at diagnosis and during open-access follow-up as required.
  • Carry out holistic needs assessments at diagnosis, end of treatment, and during open-access follow-up when necessary, providing a personalized care plan accordingly.
  • Offer treatment summaries and health and well-being interventions to all patients.
