Job Description

Our dedicated team in Liverpool are recruiting for a new and exciting opportunity as a Hospital Link Worker, to join them in providing high quality recovery care and act a liaison between local hospitals, whilst acting as a representative for our diverse client group at WithYou. If you are an enthusiastic and creative individual and are passionate about changing people’s lives for the better, we’d love to hear from you.

As a Hospital Link Worker, you will work with our clients and the local hospitals to establish and maintain a high quality hospital liason service for those suffering with substance misuse health needs. You will work closely and collaborately with the our Clinical Lead and the hospital's safeguarding team for any clients who will need extra support. This is an integral role for With You - as a frontline worker, you will deliver continuity of care through effective partnership working, care coordination, delivery of interventions and key work responsibilities. You will manage an active caseload of clients, connecting quickly with the people we support, treating them respectfully and with dignity.

You will be a compassionate and motivated individual, with an exceptional ability to build meaningful relationships. You will be highly organised, with excellent communication skills and able to manage your time effectively, working as part of a team to ensure a high quality service is maintained.

Required Skills

To be successful in this role, you will:

have attained ONC/NVQ level 3 or be willing to achieve this within 12 months have some experience of working in a health, social care or related setting have a successful track record of working with those who struggle with substance misuse have the ability to build and maintain effective relationships with clients and colleagues alike be able to take basic health care checks such as blood pressure, pulse, height and weight be able to work effectively in a team, supporting others when needed be resilient under pressure and able to prioritise you work  have good oral and written communication skills be comfortable using IT and digital media be organised and able to manage your time effectively able to be flexible with your working hours on occasion be happy to support other With You locations when needed

You will also need to meet our safeguarding requirements of a satisfactory enhanced DBS check
