Connect2socialwork are urgently looking for an Adults Service Manager to join their great Adults Hospital Discharge Social Work Team's. Paying Up to £450 a day, this role is on a locum basis for an initial 12 month rolling contract.

Role Responsibilities:
The Service Manager will be responsible for leading a city wide hospital discharge service working closely with system partners to reduce ength of stay in acute settings and promote the home first ethos.
The post holder will support in the development of the Transfer of Care Hub and Pathways 1, 2 and 3.
You will work collaboratively with service users and carers groups and staff at all levels within the service and partner organisations to develop effective reablement and strength-based services that focus on improving the outcomes of people and achieve best value.Requirements as a Social Worker include:
SWE Registration
Eligible to work in the UK
Drive and have access to a car
Post-qualified experience
Degree level or equivalent in Social Work
