Job Description


  • Care Coordinating for allocated caseload, assessments, administering medication, writing tribunal reports, progress notes, key working, CPA and CTO reviews, care act assessments, social care support, care plans, risk assessments, work under pressure, organise own caseload.
  • Ultimately, you will have a compassionate nature, be knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness.


Requirements
  • Qualified Community mental health nurse/RMN, Knowledge of RIO, experience of completing mental health assessments, Care Act assessments, interpersonal skills, communications skills, advocacy skills, documentation skills
Develop and coordinate healthcare programs
Communicate between staff, patients and family
Handle patient case management and education 
Recruit and train staff and create schedules
Develop patient goals and monitor progress
Maintain patient confidentiality and quality care
