We are currently recruiting for a Multi-Disciplinary Pathway worker/CAMHS Practitioner who is enthusiastic, strong with a knowledge of working with children and adolescents with mental health and significant behavioural difficulties.

**Responsibilities**:

- Providing comprehensive mental health assessments and formulating care plans for children and young people, taking into consideration their individual needs and circumstances.
- Collaborating with a multidisciplinary team, including psychiatrists, psychologists, and social workers, to develop and implement a coordinated treatment plan.
- Delivering evidence-based interventions and therapeutic support to children and young people, promoting their emotional well-being and resilience.
- Liaising with families, carers, and external agencies to ensure a holistic approach to care and support.
- Monitoring and evaluating the progress of children and young people, adjusting interventions and care plans as necessary.
- Ensuring accurate and timely documentation of assessments, care plans, and interventions.
- Providing consultation and support to colleagues, promoting best practices in working with children and young people.

**Shift Pattern**:

- Mon - Fri 9am - 5pm

**Applicant requirements**:

- Registered Mental Health Nurse(RMN) with a valid Pin
- Experience working with children and young people within a community mental health service
- Strong knowledge of child and adolescent mental health assessments and care plans
- Ability to work autonomously and make clinical decisions
- Strong communication and interpersonal skills
- Ability to engage with children, young people their families and external stakeholders
- Dedicated and committed
- Eligible to work in the UK
- Live in travelling distance to the area
