About the Role
We are seeking a highly skilled and motivated Associate Practitioner to join our Community Pulmonary Rehabilitation team at Sheppey Community Hospital. As a key member of our team, you will play a vital role in delivering high-quality patient care and contributing to the development of our pulmonary rehabilitation program.
Key Responsibilities
- Ensure accurate and confidential handling of patient information in accordance with Caldicott and Information Governance Policies.
- Participate in the ongoing development and evaluation of the pulmonary rehab pathway, working collaboratively with multidisciplinary teams to improve patient outcomes.
- Conduct clinical observations, including vital signs and blood tests, as requested by registered clinicians or GPs.
- Communicate effectively with patients and healthcare professionals to ensure seamless care coordination.
- Deliver prescribed pulmonary rehabilitation programs independently, providing personalized support and guidance to patients.
- Provide accurate and timely updates on patient progress to qualified practitioners.
- Evaluate patient treatment progress and provide consistent feedback to qualified practitioners.
- Manage sensitive and emotive situations with patients, utilizing effective verbal communication skills.
