Centralto the role are strong communication and organisational skills, provenspecialist clinical skills in heart failure management and experience of linemanagement and clinical supervision.

As the teamworks across multiple sites to deliver patient care, candidates must have afull driving license and access to a car for work purposes.

This is a part time role for 30 hours per week.

Main duties of the job

The post holder will work with the Heart FailureTeam Lead in thedevelopment and co-ordination of the Heart Failure Nurse specialist servicewithin the community, supporting on the integration of remote monitoring intocare pathways including virtual ward care for heart failure patients and theintegration of specialist nursing into cardiac rehab programmes.

Thepost holder will be responsible for a defined patient caseload, initiatingtreatment plans, risk factor management, chronic disease management, providingnursing care, support and education to patients and their carers.

Thepost holder will need to establish and maintain links with healthcareprofessionals, managers and support workers within both primary and secondarycare.

Job description

Job responsibilities

The postholder is responsible for working collaboratively within Community HeartFailure Specialist Team in the development and co-ordination of the Heart FailureSpecialist Service within the community, applying expertise to enable patientswith Heart Failure to receive advanced specialist care in their own home and inthe community.

The post holder will need to establish and maintainlinks with healthcare professionals, managers and support workers within bothprimary and secondary care to support patients on discharge from hospital aswell as implementing care pathways to support patients to live well with heartfailure and providing early intervention to avoid hospital admission due toexacerbation.

The post holder will be responsible for a definedpatient caseload, initiating treatment plans, risk factor management, chronicdisease management, providing nursing care, support and education to patientsand their carers.

The Heart Failure Nurse Specialist will liaise withother members of the Heart Failure Management Team in primary and secondarycare to deliver evidence-based care to patients with heart failure and workwithin the First Community Heart Failure Team to integrate remote monitoringinto daily clinical practice and monitor effectiveness and efficiency of thetechnology on the caseload.

The post holder will work with key members ofprimary and secondary care to provide education, advice and support to patientswith heart failure and will support the Cardiac Rehab Service in enablingpatients with heart failure and other cardiac conditions to improve theircardiovascular health.

The post holder will be responsible for educationand service development within the heart failure team and act as a resource foradvice for healthcare professionals, patients and carers.

Qualifications

Essential

Registered Nurse (degree level) Recognised cardiac nursing qualification. ILS provider certificate Recognised teaching qualification Holds or is working towards extended/supplementary prescribing qualification

Desirable

Evidence of study at higher education level Recognised training undertaken in motivating behaviour change Recognised counselling qualification MSc or working towards.

Experience

Minimum of two years specialist community Heart Failure nursing experience Experience of leading a small team Experience of clinical governance A minimum of 4 years post registration experience in clinical cardiology 1 year at AFC band 6 Experience of working across professional boundaries Experience of audit and research Autonomous practitioner
