An excitingopportunity has presented for a motivated and enthusiastic Care Coordinator to work within a community mental healthsetting working alongside our primary care colleagues.
JOB SUMMARY:
To act ascare co-ordinator / lead professional within the CPA process for service-usersin North East Lincolnshire. Thisinvolves being responsible for assessment, developing, implementing andevaluating care plans using a person-centred approach.
Ourideal candidate would be a committed team player as well as being able to workindependently demonstrating initiative and passionate in promoting good qualitycare to maximise independence. Within this role you will gain training,support, and guidance to ensure you are working collaboratively with externalagencies to give the best possible care for our service users.
Main duties of the job
This rolerequires someone who can manage in a fast paced environment completingassessments to support diagnosis. The day to day work for this role will be ledby the needs of the service users and the service which will require effectivetime management and prioritisation skills as well as effective communicationand autonomous working.
Allof our staff are an integral part of the multi-disciplinary team within mental health services. The successfulcandidate will need to understand the importance of engaging with externalagencies within our area in order for our service users to achieve high-qualitycare that we in Navigo always strive to deliver. This should involve usingsupervision and reflecting on practice to develop new and improved ways ofworking.

Job responsibilities
MAIN DUTIES RESPONSIBILITIES:
Communication& relationship skills
Thepost-holder will work with all members of staff within PCN's and Navigo
They willwork in close liaison with health and social care professionals in statutoryand non-statutory agencies to facilitate and deliver a range of services toensure that continuing care needs of service users are met.
Participatein multidisciplinary meetings, case conferences and any other meetings asnecessary.
Effectivelycommunicate complex and/or sensitive information where there are barriers tounderstanding. This includes communication concerning adult protection, abuse,cognitive impairment and other risks surrounding a service users mentalhealth. Barriers to understanding include working with service users who couldbe hostile or highly emotional, who have complex needs and may be vulnerable.
Maintainaccurate clinical records according to professional standards.
Toparticipate in Practice and Management supervision.
Promote andimplement creative ways of working with service users
Instigate,develop and maintain a relationship with the service user and/or carer to facilitatethe accurate delivery of care.
Managehighly confidential information in respect of members of the public inaccordance with local and national policy.
Act asnamed worker responsible for specific interventions.
Knowledge,training and experience
Attendappropriate training as identified with supervisor through supervision, reflective practice, handover, appraisal and provide feedback to the Team ontraining.
Maintainand update skills/knowledge relevant to the post and ensure practice isevidence based.
Attend andparticipate in Team and clinical meetings as required.
Participatein clinical supervision process, in appraisal and individual performance reviewprocess.
Participatein the training, education and induction of new Team members, seconded workersand other agencies as identified by Team Leader. To demonstrate and promote tothem high standard of care and working at all times.
Participatein audits/surveys.
Havespecialist knowledge and training, gained through formal training andexperience, relating to older people and mental health and apply this knowledgewhen delivering care to service users.
Provideadvice and support to other agencies around issues relating to Mental Health.
Analytical& judgement skills
Undersupervision make decisions about the range of options for treatment and careplans based on judgement of service-users need.
Conductcomprehensive assessments of both service users and carers in line with teamoperational policies
Planning& organisational skills
Undersupervision to prioritise, plan and coordinate own workload and activities inline with the changing needs of the team and service. This includes monitoringservice user medication, making appointments for service users, conductingassessments, managing emergencies and enquiries, responding to crisis workwithin individual caseload, ensuring patient records are up to date andsupporting colleagues on a daily basis.
Have theability to work independently and to be able to seek assistance
Crisisresponse to other staffs service users in the event of staff absence, anddelegation from senior staff member.
Participatein caseload management and ensure relevant information is available for this totake place.
Physicalskills
Be trainedin control and restraint and breakaway techniques; undertake regular updates(RESPECT).
Responsibilitiesfor patient/service user care
Participatein programmes of care for service users and act as associate worker for serviceusers on CPA and non-CPA, ensuring links with Care Management and Primary Care.
Reviewservice users clinical risk on each visit, reporting back any areas ofconcern/change in presentation.
Provideadvice to service users, their carers, relatives and other professionals onneeds relating their difficulties and mental health. This includes a range ofhealth education and advice.
Initiateindividual carer support where required. Set objectives, monitor and auditeffectiveness.
Participatein clinical governance activities.
Responsibilitiesfor policy & service development implementation
Adhere toall trust policies and legislation relating to the role.
Contributeto the evaluation and development of the service.
Comment onpolicies, procedures and developments where required.

Accuratelydocument episodes and information relating to service user care in patientnotes, using agreed methods of documentation. This will be done in accordancewith Navigo policies, professional guidelines and data protection legislation.
Recordpersonally generated information as required by the team or service, asdirected by the Team Manager/Leader.
Provideoral/written reports as required.
Managehighly confidential information in respect of members of the public inaccordance with local and national policy.
Ensure thatthe requirements of the Data Protection Act 1998 are met.

Workdirectly with difficult family situations and circumstances. For example,communication with service users, families and other agencies around receivinga diagnosis of dementia, adult protection, suicide and overdose, and othermental health issues.

Person Specification
Experience
Essential
Experience of working with all age functioning adults Experience of working within a CPA framework. Experience of liaising with other teams or services, for example GPs, voluntary sector services.
Desirable
Involvement in innovative nursing/social work practice. Worked as key-worker/care co-ordinator or equivalent. Experience of taking part in recruitment and selection processes. Experience of care management role.
Additional Criteria
Essential
Knowledge of assessment, planning, implementation and evaluation of individualised care plans. Evidence of knowledge of current professional issues. Relevant professional Mental Health legislation/guidance. Basic theoretical concepts in nursing/social work. NICE guidelines, NSF, Clinical Governance. Must hold a full driving licence and have access to a car for work purposes.
Desirable
Awareness of current research in the field. Knowledge of Person Centred Care Knowledge of audit process.
