A new team has been created in West Place and we are looking to transform the face of Community Nursing. The Unplanned Care Team has been created to support unplanned nursing care and to run catheter clinics in the community. The applicant should have excellent communication skills with a good understanding of IT systems. Previous Community Nursing experience would be beneficial for this position.

We are looking for a well-motivated, team player who will be expected to work in a busy environment and be able to meet the demands of this new Service. Support and training will be provided to ensure competency and skills are reached, maintained, and future development encouraged.

Main duties of the job

To undertake unplanned care visits, to support the Community Nursing teams across West Place. To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by assessing, planning, implementing and evaluating packages of care.

Role will aim to be approximately 20% Triaging incoming community nursing referrals, assessing the patient's needs, determining the priority of care, and processing the referrals accordingly.

About us

This is a new team working within Triage in West Place, consisting of Triage Nurses, and allocators led by a Clinical Operations Manager. We endeavour to provide excellent face-to-face urgent care to patients to ensure safe, relevant and timely care is provided to the local housebound population of West Norfolk. We work closely with our Community Nursing, GP and Social Services colleagues.

Job responsibilities

Key Areas of Responsibility

1. To undertake unplanned care in conjunction with community nursing teams.

2. To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.

3. To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.

4. To support community nursing teams with provision of care in catheter clinics

5. To work within the integrated team to facilitate early discharge from hospital.

6. To work within the integrated team to prevent unnecessary admission to hospital.

7. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

8. To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.

9. To document triage notes in order that the patients record is complete and for audit purposes.

10. To use information technology to process the referrals for the 20% triaging role.
