We are offering a part time ( hrs per week) 12 month secondment opportunity for achildren's nurse (or a general/Learning Disability nurse with significant childrensexperience) who enjoys working withchildren/young people and their families where there are complex health needs.
This role would bridge the gap between children and adult teams withinthe Continuing Healthcare Team and ensure good transition practices werechampioned. This post would also include the assessment and caseload managementof children and young adults who receive NHS funded care.

Main duties of the job
Triage and assessment of Children and Young Adults who have beenreferred for Childrens Continuing Care / Continuing Health Care ensuringNational Framework compliance.
Review of care packages and eligibility using the CCC/CHC National Frameworks.
Caseload management of children with complex health needs and youngadults with complex learning disabilities who receive NHS funded care.
Joint working with the clinical team, care providers and other agenciesaround the child/YP and their family, including social care and education andthe voluntary sector.
To ensure that the transition between children and adult services withinNHS Somerset and in the wider NHS is smooth and seamless and transitionpractices are adhered to and continually improved.
To strive to ensure any care provision is centered around thechild/young adult and personalised to meet their holistic needs and achievetheir outcomes wherever possible

Job responsibilities
PURPOSE OF THE ROLE:
The post holder will undertake all Children's ContinuingCare (CCC) and Continuing Healthcare (CHC) processes and assessments onindividuals who may be eligible or who may already be in receipt of funding.
They will provide contact with patients, carers andfamilies.
The post holder will be expected to work across children and adultprovider services and develop strong links with clinical services staff acrossthe trust, as well as colleagues from external organisation such as SocialServices, Education, Acute Trusts, other ICBs, independent contractors andprivate care homes and providers.
KEY RESPONSIBILTIES OF THE ROLE :
To undertake Children'sContinuing Care (CCC) and Continuing Healthcare (CHC) processes and assessmentson individuals who may be eligible or who may already be in receipt of fundingthis will include those in receipt of fast track funding at end of life andthose in receipt of a Personal Health Budget
To case manage individualsin receipt of NHS funded care and support, reviewing and reassessing needs, theservice provision and support required, this will include those in receipt of aPersonal Health Budget.
To provide and receivehighly complex, sensitive and contentious information, communicating unwelcomefinancial decisions to members of the public and using interpersonal skills toovercome barriers to acceptance where there may be hostility and antagonism.This liaison is an integral part of the role and will take the form of regularcontact by telephone, face to face meetings, written and electroniccommunication.
To ensure effective timelycommunication and liaison with key stakeholders, including partner agencies,patients and their families, and care providers.
To be actively involved inthe PHB process including setting up PHBs, reviewing and monitoring of PHBS,and the development of Commissioning Care Plans for providers. Support the achievement oftimely delivery of service, to work to local policies, systems and processesaround CCC, CHC, Fast track as directed with reference to the relevant keynational and local priorities
To review any service, andcare, commissioned in regards to quality and appropriateness to the individualsneed, to ensure that the provider is delivering the requirements of thecontract ensuring value for money in all matters relating to packages for continuingNHS healthcare.
To ensure familiarity withthe children and adult National Frameworks for funded NHS care and maintainconsistency in the application of the national and local policies oneligibility for CCC and CHC.
Be confident in presentingcases to Children's panel and to MDT meetings and making recommendations oneligibility and the care/support required.
Responsible for providingfirst level contact for disputes/appeals/resolution meetings between partneragencies or with patients / families / legal bodies, for example followingunwelcome financial decisions.
To attendappeal/independent review panel meetings/ Choice & Equity panels andpresent cases as required. To be able to identifysafeguarding and quality concerns, maintain full awareness and stay updatedaround child protection issues and alertness to potential vulnerable youngadults and take appropriate action
To be able to apply theMental Capacity Act (2005) appropriately and have a sound understandingDeprivation of Liberty Safeguards (DoLs) (and Liberty Protection Safeguards) To actively participate inBest Interest Meetings, Safeguarding enquiries, Child In Need meetings, caseconferences or other relevant meetings as necessary Support the Team Leadersand managers in investigating and responding to any complaints.
Act as an expert CCC/CHCresource for young peoples complex health care and learning disabilities tohealth and social care workers, patients, families and other organisations andindividuals as appropriate including assisting with the development and deliveryof training within the CCC/CHC department and externally.
Support and undertake auditand research relating to CCC /CHC activity
To ensure compliance withall statutory and mandatory training.
