Purpose of the role Works as part of the multi-professional SEND Health Team to ensure that the health needs and provision are outlined clearly in Education, Health and Care Plans (EHCP) in Somerset. Work with provider health organisations and the Local Authority to improve the experience and effectiveness of the EHCP process including providing clinical advice when agreeing wording in plans. Work with the DCO and deputy DCO to ensure that Somerset Integrated Care Board (ICB) meet their statutory duties for children and young people with Special Educational Needs and/or Disability (SEND) aged 0 25 years. Duties and Responsibilities Communication and Key Working Relationships Develops positive working relationships across provider health organisations and the Local Authority casework team and within the Health SEND team.

Plans and delivers specialist training to staff in relevant agencies to promote the understanding of the EHCP process from the health point of view. Provides specialist clinical advice, training and support to Local Authority officers regarding the health element of children and young peoples EHCPs. Communicates empathetically and clearly with families of children and young people who are undergoing an EHCP assessment or who have an existing EHCP plan. Liaises with colleagues working in SEND in other areas of the country to establish networking channels to share and learn from experiences and good practice.

Represents the SEND Health team at appropriate meetings, taking actions and reporting back to the wider team. Actively seeks resolution of disagreements with parents and other professionals using good clinical and communication skills to try to attain a positive outcome. Planning and Organisation Plans, manages and prioritises own time independently. Works to frequent deadlines, planning ahead and flagging up lack of capacity in a timely way so assistance can be provided.

Able to complete longer-term projects alongside more routine weekly tasks. Has a role in ensuring that the health contributions to EHCPs are completed within the set timescales. Supports the SEND administrator in dealing with queries and ensuring the administration related to the role is completed to correct timescales. Analytics Provides accurate and timely statistical data.

Assists the DCO in audits of the health element of EHCPs. Participates in setting up and maintaining quality assurance systems. Contributes to the gathering of data to influence commissioning decisions and predict future trends. Responsibility for Patient/Client Care Prepares for and attends weekly multi-agency Statutory Panel meetings to advise on the health element of children and young people who are or may undergo an EHCP assessment.

Ensures the involvement of parents and young people whenever possible when making decisions about their care. Highlights unmet need to DCO to inform discussions and actions to improve the quality of service provision. Maintains professional registration, ensuring continuing professional development in the role. Policy, Service, Research & Development Responsibility Adheres to NHS Somerset ICB policies and protocols and national and local standards and guidelines relating to professional practice.

Deals with initial complaints sensitively and passes on to line manager at the earliest opportunity. Is aware of and adheres to current legislation and employer procedures relating to the post. Attends and contributes to staff meetings. Attends all mandatory training as required by NHS Somerset ICB Contributes to research, as appropriate, and participates in departmental research design and implementation.

Participates in service evaluation and development initiatives within the SEND team to better deliver the Code of Practice in Somerset taking on delegated pieces of work reporting to the DCO Responsibility for Finance, Equipment & Other Resources Advises the DCO of any financial implications of cases involved with, ensuring seeking sign-off if any additional expenditure is required to meet the health needs of the child or young person. Responsibility for Supervision, Leadership & Management Supervises the work of the SEND Administrator when required. Information Resources & Administrative Duties Accesses patient record systems as required maintaining high standards of confidentiality and complying with all information sharing protocols. Deals with initial complaints sensitively and passes on to line manager at the earliest opportunity.

Is aware of and adheres to current legislation and employer procedures relating to the post. Any other Specific Tasks Required The post is office based with requires the post-holder to sit in a restricted position for the majority of the working day.
