Job summary

Long Stratton Medical Partnership is looking to recruit an advanced nurse practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. This is a part time post of 30 hours per week, the days of work will include a Monday and Friday, the other days will be discussed at interview.

The ANP will be expected to conduct their clinical practice within the surgery setting or whilst visiting patients at their place of residence. Furthermore, the post-holder will participate in the practice rota for duty clinician.

Main duties of the job

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and Practice protocols, and within scope of practice Initiates appropriate diagnostic strategies and therapeutic interventions including radiology and laboratory requests, with consideration and attention given to safety, acceptability and efficacy Interprets and evaluates diagnostic test results in liaison with the patient, carers, family and their GP Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interaction.

Long Stratton is a small town in South Norfolk, 10 miles from the historic city of Norwich. We have 11,500 patients on our list, a CQC rating of Good and operate from both our main site and a branch site in Newton Flotman. We are also a dispensing practice with a dispensing list of around 5,500 patients. We use EMIS Web and run cloud-based telephony which links seamlessly with the clinical system and provides excellent functionality.

Our practice has 4 partner GPs leading an MDT of 5 permanent salaried GPs, an emergency care practitioner, an advanced nurse practitioner, 2 clinical pharmacists, a pharmacy technician, a team of 6 registered nurses and 3 HCAs plus superb teams in reception, administration, and dispensary, along with the surgery management team.
