Job summary
This is an exciting opportunity to be part of a new organisational structure under a new management system. You will work as part of a new outward facing Primary Care Team in an organic culture, embracing change and flexibility within the new Gordon Street surgery site. Your line management will be split between the Operational Lead who will manage your reception function and the Care Coordination Lead who will manage the new care coordination function within the organisation.
You will work within a team of receptionists / care co-ordinators to receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous,efficient, effective and confidential way.
You will provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone
You will ensure that the practice runs smoothly and assist with other back office role provides a high level of service to patients, doctors, nurses and other members of the Practice team.

Main duties of the job
You may be asked to undertake both reception roles and work in a flexible way rotating between reception and care coordination as we are looking to work in new and innovative ways.
You will work with the ESPCP team to ascertain how such flexible working could function whilst optimising the care coordination pathways.
You will ensure that the practice runs smoothly and assist with other back office administration. This role provides a high level of service to patients, doctors, nurses and other members of the Practice team.
You will work within a team of receptionists / care co-ordinators to receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous,efficient, effective and confidential way.

On the 7th May 2024, East Staffordshire Primary Care Partnership were awarded a caretaker contract for the service provision of Gordon Street Surgery. More commonly known as East Staffordshire PCNwhich sits under the legal entity of East Staffordshire Primary Care Partnership Ltd (ESPCP) which is a legal entity for offering authorised NHS employment and delivering NHS services.
ESPCN comprises a network of 18 GP practices across East Staffordshire with a population of 150,000 patients.
The management team has extensive experience of Executive Leadership, commercial, finance and management support, practice management, specialised digital and IT, HR and recruitment.

The following are the core responsibilities of the Receptionist / Care Coordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.
Reception: Ensure an effective and efficient reception service is provided to patients and any other visitors to the practice. Deal with all general enquiries, explain procedures and make new and follow-up appointments. You will learn how to read code and process workflow accordingly this will form part of your administrative function. Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, including patients from abroad and ensure procedures are completed. Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt delivery. Administration: Enter home visits into the visit book. Issue repeat prescriptions. Advise patients of relevant charges for private services. To action referrals from the clinical team. Open and secure the premises at the start and the end of the day. Care Coordination: Work with a cohort of identified patients to coordinate and navigate personalised care. Help patients to manage needs, answering questions and supporting to make appointments. Ensure that patients have good quality information to make choices about their care
