Job summary
Discharge Co- Ordinator Band 4 - hours
The post will support wards to safely plan and discharge patients with the support required.
Discussions with patients and family members and the ability to use and refer to specialists teams in the community as required
Attend daily board rounds- to be aware of all the patients on your allocated ward area.
To liaise with the Band 6 supporting your team
We will support with any development staff member needs
We provide in house training relevant to our area .
We can access training from community teams
Regular supervision
Yearly appraisal
Main duties of the job
The role of the discharge Co-ordinator is to provide support to wards, attend board rounds as discharge starts on admission . Ensure that all patients are discharged safely with the care they require. To be able to escalate any delays in care or discharge planningTo liaise with other team members for patients from care homes and patients that are approaching end of life

Ability to be an effective advocate for the patient within a multi-disciplinary team setting. Demonstrable oral and written communication skills (listening; expressing; summarizing; enquiring) Able to create/amend documents in word processing formats; excel spreadsheets, presentations and email. Ability to undertake health care and risk assessments Able to effectively manage own time and work load Problem solving skills. Ability to deal with conflict or sensitive situations Communication skills which facilitate discussions with patients, relatives, and clinical and managerial staff at all levels. Presentation skills in a formal or informal setting.

Essential
Ability to travel to varying work locations at varying times and during unsocial hours if needed Physical ability to access all clinical ward areas, and may need to work off the main site. Able to work weekends and Bank Holidays or unsocial hours on a Rota basis 7/7.-
